ABC Healthcare utilizes Patient-Centered Medical Home to administer patient-centered treatment to patients. It has a very high reliance on patient care assistants (PCA) who aid by offering information that educates patients and works with other staff who are involved with the patient care team. The work of PCA’s requires constant information reviews and updates. This information is used to aid in the provision of the proper management of diseases by the patient through guidance with the patient care assistants. ABC Hospital saw the need of automating the PCMH to provide efficient services to patients. PCMH works by offering medical assistance where the patient is there is a need for coordination with pharmacist’s ad people near the patient to enable quick medical assistance when required by the patient.

PCA’s need to record the data they get from conducting tests which check the patient’s blood pressure, sugar levels and cholesterol levels. ABC Hospital’s data captured by the eHealth system are complete; the data obtained by the system covers all areas; it covers the patient’s details, the medical reviews, the type of disease a patient suffers from and their bill. Despite the fact that the data is complete, there have been some discrepancies in data accuracy. ABC had had a problem with providing the correct data about clinical conditions, health status or population demographics when data was pulled by the use of billing codes. ABC can also develop reports that will show the number of patients' suffering from a certain illness; they can run a query to bring the total number of patients with High blood pressure or diabetes.
