Root cause analysis of an interoperability issue Name Institution Professor Course Date Challenges of data sharing and communication gaps are persisting problems in the healthcare between different care providers various players and even different departments within a health system. Despite the future promises of technology on network connectivity and free-flow of information the issue of interoperability continues to pose a challenge in delivering optimum patient care (Zhang & Xu 2006). Lack of interoperability in healthcare can lead to a serious issue for patients. For instance the main cause of medication errors is assumed to be the communication breakdown. Most of the medication errors are estimated to occur during the times of information transition. As a result this may put a patient at a risk for a poor outcome which may include re-hospitalization (Construction 2007). Although interoperability may not always enable more coordinated care health IT is root cause analysis to determine the root cause of interoperability problem. After identifying the root cause of the interoperability issue it is important to work on it so as to prevent future problems. The root cause of the issue can be identified as the poor communication among healthcare stakeholders. The best practices to avoid such pitfalls and maintain sustainable HIE in future include technological improvement data governance improving finance sustainability and technical operations as well as technical infrastructure.
