Through the 24th personality, Kevin becomes a kidnapper of young women who becomes the food to his personality, the personality which happened to be the strongest of all the other personalities making him a very powerful being, with massive strength, and having the ability of doing things which seem impossible to normal humans like growing in body and muscles within a very short moment. The movie progresses with the girls trying to making attempts to escape out of his confines with the use of the other less harmful personalities. Kevin could be diagnosed with a disorder which is well known as multiple personality disorder which according to the DSM-V is known as a dissociative personality disorder. This is a condition that an individual does not reside in one personality, as it should be in a normal sense, but dwell in two or even more personalities as shown by Kevin.

It is found that each and every one of these personalities possesses different personal characteristics which include different preferences, interests, age, sex, different walking behaviors, the way of conversing with other people among others. According to psychologists, flood confusion is one of the common symptoms possessed by the DID patients since they become victim to knowledge confusion and reality. Because Kevin is also not able to remember what happens when other personalities take the course, the director's depiction of Dissociative Identity Disorder (DID) is an accurate confirmation to ICD-10's major criteria of DID diagnosis. Dissociation is often one of the common characteristic or a symptom of DID thus its name. What causes Dissociative Identity Disorders? Even though the explanation from the film might far surpass the clinical explanation, the only theory which remains acceptable is that it is a mechanism of elusion prior to a trauma or a very intense form of abuse at early life stages.
