Executive summary The continuum of care is a health care system that entails tracking a patient over a period of time through several different health services in various levels and depth of care. Ambulatory care also known as outpatient care is medical service received via outpatient methods. Ambulatory care has been greatly enhanced through developments in medical technology where procedures can be even be done even outside medical facilities. Ambulatory care can take place through Smartphones computers and the internet converted buses and trucks which facilitate outreach to where the patients can be found retail clinics and work-based clinics and high tech centers that are geared towards specialized care depending on the target population. The strength of the services based on Smartphones computers and oversight and guidance so that ambulatory services particularly those owned by for-profit firms do not o off the rails and become the mainstay of the medical services sector. Patients still require an effective system that can relate one sector to another. Ambulatory services can lead to a fracturing of the health system if not properly managed (Chang et al. 2015).
