In the community there are public health issues that are identified to contribute to poor health outcomes in the community. Such issues include drug use and on community interventions and reports from previous programs in the community of interest and from other similar community interventions in different communities. There is the community-centered timing of the process depending on the size community needs available resources which include the available timeframe for the specific community intervention. Select one Healthy People 2020 leading health indicator (LHI) that relates to your family's community. Based on your selected LHI topic: • Identify at least one nursing intervention for each diagnosis above. • List the health partnership(s) that could or do assist in implementing the interventions in the community.
