Heart Disease Nursing Care Plan Carl Edwards is a 75-year-old man with congestive heart failure. Having sustained three myocardial infarctions in the last 10 years he has decreased left ventricular function. Mr. Edwards takes Digoxin Capoten Coreg and Lasix for management of this disease. Today he presents to the emergency department with fatigue generalized weakness and feelings of “skipping” heartbeats. Upon arrival he is placed on the cardiac monitor his vital signs are assessed and an IV is inserted. He currently denies chest pain but is experiencing some shortness of breath and is placed on 2 L of oxygen via nasal cannula. Nursing Care Plan A. Medical Diagnosis- Heart Failure b. Definition and Etiology- heart failure is a condition where the heart is pumping blood properly leading to inadequate blood and oxygen supply to the cells to meet metabolic demands of the body. It is chronic condition is 0.125-.25 mg daily. Morphine Eases myocardial workload. Reliefs some of the side effects of other medication. Dosage is 2.5-10 mg every four to six hours. Isordil Increases coronary blood flow. It also increases myocardial perfusion. Side effects include dizziness nausea headache and flushing. Administer 2.5 to 5mg twice a day. Furosemide (Lasix); spironolactone (Aldactone) Decreases edema formation and diminish afterload. Side effects include jaundice diarrhea nausea and constipation. Dosage of 20-40mg every eight hours. Squatting or knee-chest position Education The patient and his family will be educated about adopting a low-sodium diet proper medication management engaging in activity and exercises and how to recognize symptoms of deterioration. Evaluation The patient will be evaluated for adherence to self-care regime. The patient will be evaluated to ensure that he has the right fluid balance on the left side.
