Weight to height determines the weight of a child. Children under five years who have more than three weight to height ratio are considered obese. Percentages of obesity problems are greater in the mental health and wellbeing and learning disabled population (PWLD) than the general population. This report is according to recent research. The disparity in the number of people affected by obesity is connected to social and economic factors. Some researchers attribute this to the fact that some of them could be having secondary disorders. However, a certain percentage of the health inequality cannot be account for, below are some factors that contribute to mental and physical health inequalities in PWLD Social gradient is one of the factors that cause the health disparity.

In the case of a country, a developing country will have one of the weakest health recorded. This is a phenomenon seen in developing and developed countries. In the case of PWLD, most of them have a secondary disability this, therefore, means they are dependant on the government to provide basic needs including health care or on their families. Unemployment is another factor. It is tough for people with mental illnesses and learning difficulties to get employment. Research shows most of this people have other disabilities that means that they cannot efficiently deliver in work places. This people result in doing small businesses, begging in the streets or even entirely depending on the family. What they get, is not enough to meet basic needs like food and healthcare.

They think they are of no benefit; some think they are just a burden. Social discrimination and exclusion of people with mental disabilities increase their chances of depression; depression worsens their health state. When they are loved, they tend to develop better esteem and hence live above their limitations. Showing love to them helps them build confidence and explore their hidden potential. Caring for them and supporting their needs is a sign of love. The early life of a child is considered one of the most critical phases in their development. This includes the physical aspect, cognitive domains, and emotional domain. Most of the children with learning and mental disabilities are raised under isolation and social exclusion. A child's brain develops in the early years.

Children brought up in an unstable background, negligence and maltreated increase the chance of poor health conditions. In extreme mental cases, the individual is not able to make a rational decision about their treatment. The court comes in hand to intervene in such affairs. Policies are guiding involuntary treatments. Family members act as carers of the patient. When the patient condition is at extreme levels, the carer helps in making decisions on their treatment. They protect them from being exploited through their set laws in the bill. This has helped PWLD access freely public services like all other people(Potter, Frances Fanny Elizabeth 43). Apart from the American with disabilities act, there are professional, ethical and legal principles and rights that protect service users, carers and the general public.

These principles are found in the mental health act 2014. This principle must be upheld and respected by every person. They should not take advantage of their vulnerability (Walker, Carol, et al. Careers (including families) for those suffering from mental health disorders should have their efforts identified, respected and appreciated. People with mental disorder should be permitted to make decisions on their evaluation, treatment, and procedures that are risky. They should be provided relevant information that can influence their decisions Liberty and security-Everyone has a right to personal freedom. This freedom cannot be wholly taken away. These people depend on learning disability nurses who use specialized skills to care for them. Learning disability nurses seek to reduce barriers facing this kind of people.

With a relative increase of people with mental illnesses, there is need to train more learning disability nurses. Nursing process Assessment process-This is the first stage in this nursing process. It involves collecting information about the patient's physiological, psychological, spiritual and sociological status. Implementation process-This stage the nurse follows through the planned actions. The plan is unique from one person to another so is the solution. Implementation process can take place over an hour, days or even weeks. Activities involved in the implementation stage include monitoring change in the patient, doing follow-ups and also educating the patients on health management. Evaluation phase- This is the last process in the nursing. Some social factors that influence health include gender, health services, culture, food security, social support centers and education.

These factors lead to drastic differences between the people in the society. Social model strives to change all these social factors that contribute to deteriorating health in the community. This can be achieved by generating a good and comfortable environment for everyone in the society. Medical model The medical model of health promotion strives to improve the health of people. These charity organization has continued to listen to cases of preventable from the families that lost their loved ones within the national health services. They launched ‘74 deaths and counting’ report; it had detailed information of these cases. According to the confidential enquiry, two hundred and thirty five people with learning disabilities have died over the past one year. About 23% of PWLDs die when they are younger than 50 years.

Confidential inquiry provides substantial evidence on the causes of these premature deaths (Baines, Susannah, and Hatton, 56-62). The National Health Service is accused of failure to meet the needs of individuals with learning disorder (Hill, 15). Delays in treatment and diagnosis are attributed to lack of proper training of the hospitals staffs (Edwards, Judith, et al. 199-216) The confidential inquiry into premature deaths of people with learning disabilities also published their review. To the uttermost surprise the figures estimated were similar to those of death by indifference was similar (Potter, Frances Fanny Elizabeth ,43). They reported that chances of people with learning disability to have poor health are high compared to the general population so is the death. To add to that the food proportion of junk consumed is a lot.

The government has helped in the fight of obesity by authorizing a reduction of sugar level in some of the readymade food and drinks.
