Electronic medical record has many advantages; it is more useful than a paper data because it helps to keep a track of patient’s medical history. Paper data always has security issues as being able to be mishandled or misplaced by anyone. Storing paper data is also space consuming and overtime has a chance of being destroyed. Whereas, Electronic medical record, helps in data retrieval from any place . It helps in identifying patients who are due for preventive visits and also screenings. It helps in measuring patient parameters such as blood pressure readings and vaccinations. It helps in follow up and also assist new providers to get a quick access to patient medical history in case of emergency.
However, there are many challenges that are hindering in EMR to be accepted by all providers. Cost being the main issue, many health care providers has cash in hand to implement EHR’s in their practice but they are unable to calculate the cost of maintaining, training, infrastructure and support that stops them from EHR adoption.
Time and preparation is another issue, many providers specially who are working in smaller practices complain that entering and saving patient information is time consuming and involves lot of training and hence they start losing business as they say they cannot provide enough time in communicating with the patients.
Roll out strategy to determine the way of implementing EMR in a practice, either too quick adoption like a big bang theory in which all the changes are made at once which require many personnel and also lot of support or a strategy to take a small developmental curve towards adoption which takes lots of time in implementation. Choosing a right strategy is really important, if not there might be chance of failure. Unavailability of support personnel is also an issue for EMR adoption.
For the scenario presented, where a pediatric physician is reluctant to adopt EMR:
I would take the following steps to convince the physician for EMR adoption:
1) I will first effectively communicate the benefits of EMR and assure her that proper training and support will be provided to them in EMR adoption.
2) Physicians often get frustrated as most of the times the productivity falls right after the new system is installed. So I will stress the importance of EMR adoption in comparison with the documents but not stress on saving time and being useful in future as these are the long term goals after successful EMR adoption.
3)As the physician complained about the decrease in productivity, I would rather know the reason for decrease in productivity has nothing to do with EMR implementation and explain the same to her.
4) She complained of being not able to communicate effectively, this may be because of lack of eye contact with patient as they might be facing computer, for that, I would suggest them to use tablets which may not hinder in communication as they can hold it right like a patient chart that they use to do before.
5) If still the doctor is reluctant and hints of leaving the practice, I would explain her that the importance of EMR’s is known even to the patients as she deals with the pediatrics, I will tell her that she may lose patients as parents now a days wants to adopt EMR’s too as it will help them keep a track of their child’s health from the beginning.. And then assure her that proper training and support will be provided after and during the EMR adoption.
