Heart failure is a global health concern. According to Stamp et al. , (2018) heart failure is the leading cause of admission and readmission in patients who are 65 years and above. It is also the leading cause of death in the US among the patients who had been previously hospitalized. In the prevention of this condition nursing play important roles. Key elements of nursing’s participation in treatment of patients with heart failure are facilitation of transition process from pediatrics to adult cardiology depending on age of patient, they identify patients’ needs and counsel patients and families of the patient.
