The Community Health Assessment (CHA) is identified as the process of engaging the community collection of demographic survey and health outcomes information to determine the existing health disparities within the community setting. Through the CHA the organization can obtain comprehensive information regarding the current health status of the community needs and issues (CDC 2017). Significantly this information can be used to establish a community health improvement plan and the specific areas that resources should be allocated to address best the existing needs. The use of this community health assessment is associated with many benefits including increased collaboration between the organization and the community. Enhanced cooperation and coordination is essential as the organization can identify the health needs of the community (CDC 2017).
