Perioperative/Operating Room Experience

CLINICAL ROTATION: Perioperative/Operating Room Experience and Reaction Paper DESCRIPTION: Students are assigned to a one-day observational clinical experience in the Operating room/Post Anesthesia Care Unit at Trinitas Hospital. Students focus on the peri-operative role of the circulating and scrub nurse in caring for the client undergoing surgery. This observational experience assists the student in understanding the structure and function of internal organs and the impact of the surgical procedure on the internal homeostasis and overall health of the client. Students are also afforded the opportunity to observe the administration of anesthesia and to understand the interaction of anesthesia with the respiratory and cardiac functions. The experience in the PACU focuses on the care of the client immediately following an invasive procedure and recovering from the administration of anesthesia. Clients in the PACU are in an acute unstable condition manifested in part by alterations in consciousness, and alterations in cardiovascular and respiratory function. Course Outcomes: 1. utilize the nursing process in a caring manner according to established priorities in adult medical/surgical nursing practice. 2. apply legal standards, values and ethical principles in the delivery of care. 3. establish therapeutic communication skills with multicultural clients(s) and famil(ies) in diverse health care settings. 4. begin to integrate the role of the nurse in interactions with members of the health care team. 5. utilize teaching/learning principles to assist the client. 6. integrate concepts of multicultural diversity in the delivery of health care. 7. utilize current information technology and informatics in planning and implementing client care. 9. apply knowledge of pathophysiology and treatment modalities in developing a safe and effective plan of care. 10. employ critical thinking to make a competent decision in a variety of adult health care experiences. Objectives: 1. Review Perry & Potter textbook for care of the surgical patient. The night prior to going to the OR, watch an animated video with information available at http://www.surgery.com/ 2. Review the objectives for the perioperative experience. 3. Consult with your clinical instructor as to the expectations of the surgical staff. 4. During the observational time in the surgical areas, plan to ask questions of the staff at appropriate times. 5. Be prepared to present this surgical case at post conference. 6. Identify preoperative care – a) Preoperative diagnosis, consents, medications, laboratory results, and teaching. b) Role of team members: Nurse in the care of clients in the peri-operative setting. i. They should focus on the following things: ii. Client safety –Patient Identification, Universal Protocol; Skin integrity, etc.) iii. Prevention of medical errors iv. Professional judgment v. Delegation 7. Observe intraoperative care – Role of anesthesia, the surgeon, nurses, technicians, and other personnel. Collaboration amongst members of the intra-operative team. 8. Postoperative care – postop diagnosis, the role of the nurse, nursing process, and discharge planning. 9. The students shall submit electronically a typewritten 2–3 page reaction paper. 10. Surgical infection prevention prophylaxis (antibiotic), CAUTI (Foley cath removal postop), etc. Intraoperative Stage: Roles of Surgical Team Members: Incorporates the roles of the Surgical Team Members (surgeon, scrub and circulating nurses, OR techs, anesthesia, etc.). Demonstration of teamwork and collaborative management for surgical patients. Post-operative Stage: Role of the Nurse and Anesthesia in the recovery process: Management of airway, vital signs, etc. Discussion of the RN’s Role in Caring for the Perioperative Patient: Incorporates nursing care and the client’s reactions. How the nurse functions in this setting. How the nurse cares for the specific needs of the patient (pain, Evaluation of the Experience: Incorporates an evaluation of the experience and how it accomplishes the program and student learning outcomes: Rates the value of this learning activity and explain what was the most important or surprising thing you learned.
