Unlike heterosexual families, the appearance of a child in a same-sex family is almost always a conscious step and is associated with the adoption of a set of decisions. One such decision, for example, may be a decision about which of the women in the lesbian couple will bear the child and what role the biological father will perform.

There are several possibilities for the appearance of children in same-sex families. On the one hand, these are children born in previous heterosexual relationships. In this case, the second biological parent remains outside the same-sex family. Recently, the number of same-sex families, in which children are born and brought up directly in this family at the request of both partners, has increased. At the same time, the attitude towards the second biological parent can be completely different – from a complete lack of contacts to regular participation in the life and upbringing of the child.

The conditions for the appearance and upbringing of children in male and female couples differ due to the difference in the possibilities for the implementation of parenthood. Male couples can get a child only with the participation of a woman ready to bear and give birth to a child. In addition to natural conception, this can be done with the help of a surrogate mother, which is not always feasible due to the existence of various legal regulations with regard to surrogate motherhood in different countries. Thus, male couples are most often forced to adopt children. Female couples have more opportunities to have a child using donor sperm or using artificial in vitro fertilization (IVF).

The American Psychological Association supported the adoption and education of children in same-sex couples with their official statements of 2004. The American Medical Association issued a similar statement that supports the practice of adopting by same-sex couples, and encourages members to fight for the elimination of health inequalities for such children.

In 2007, sociologists from the University of Amsterdam in the Netherlands conducted a study of 100 heterosexual and 100 lesbian couples in which children aged 4–8 were brought up from birth. The number of boys and girls in the study groups was approximately the same. As a result of the research, it turned out that it is not possible to detect a difference between the level of well-being of children in these families.
