The action takes place in a psychiatric ward of a hospital in the Midwest, and the characters of the book are the patients. It turns out, however, that they are not crazy. These people cannot just adapt to life in society for various reasons. One of them (Billy Bibb) stutterer and painfully shy, the other (Harding) suffers from a sense of inferiority due to the change of his wife, etc. All of them are "rabbits" and cannot fend for themselves and the world "created for the wolves," as "Chief crazy" of Harding department McMurphy explained to the beginner. Psychiatric Clinic, described in the novel, is very modern, with easy chairs, a TV, a radio, a courteous staff and patient’s self-management council. It is a small model of the consumer society, America in general. It is not accidentally that composition of patients and staff varies in the ethnical and social sense. Here you can find Indians, Swedish, Americans, Irish, Scottish, negro-bearers and even one Japanese nurse.

Among the patients, there are old people and young people with a university degree (Harding) and without any education (George washstand). All of them are entitled to equal rights: they are well fed, lead on walks and kept clean. Many of them are aware of the fact that their "free choice" - but an illusion of freedom. But tit is a fee for the comfort and peace of mind. It is an exorbitant fee and there are daily and hourly committed spiritual murder. All the non-standard, but the real people who had gathered at the clinic, undergo a monstrous pressure, constant psychological treatment, the purpose of which alleged adaptation to the conditions of social life, but in reality it is standardization and leveling of the individual.

In order to neutralize a person first of all you must humiliate and trample it, and it is the subject to the entire department: the iron schedule, always rumbling radio, vigilant supervision of patients and group "therapeutic". Finally, this is the very threat of the use of radical means of "reposition drain" - electroshock and lobotomy.
