Client 1 (John) - Ram Loss

Observation and Customer Background My customer because of a car crash now suffers lack of memory, this happened five years back. John (The name was evolved for confidentiality reasons) is a 55 year old contractor that automobile accident made him lose his partner and boy as well as his memory. From interview conducted between your nurse and his sibling consequence of which shows the client was a family man who often continued family strolls in the recreation area and enjoyed spending time with his family. The client's sibling educated me that the John loves watching films, Television shows and walks. From the observation it appears that the client appears to have an issue with remembering people's names, brands of products and the client often mixes up labels of food products. Your client has problems with putting on clothes since he doesn't keep in mind where they are, as well as the client has difficulties with turning of kitchen appliances which is a potential hazard and intensely dangerous. Your client during strolls often forgets the path home, of an well-known area to him. The client often forgets about his medication.

Challenges The client's main challenge is the kitchen where the client appears to constantly forget about the location of meals and the name of products. The client has problems with remembering about turning off appliances for the kitchen. The biggest task for the client is going on the walk since he often forgets his way back home. This big fret to sibling and he'll like care center to asses John and find out if we can help.

Competence John has demonstrated competences to dressing up, John is open to use his own bathroom, and customer also eating and dressing up himself.

Current Options My Client needs support in the kitchen and assist with the preparing of meals, John needs assistance with walking.

Realistic Recommendation To improve the lives of your client I suggest to match John with a wristband with a built -in Gps device tracking, just in case if John lost, will be easier for the family to find John, but I hardly recommended HealthCare Associate for assist John in day walk. I also recommend putting stickers with the brands of food products, with the hope that such stickers can help the customer.

Personal Learning Loss of storage is something bad and it can occur to anyone folks. With my observations and research, I grasped what my client needs every day, and I learned how to help him as I can. Client 2(Henry) - Physical Disability

Observation and Customer Background

My client Henry, 44 yrs. old throughout a walk in the park had a stroke.

From the interview conducted with the client's family, it appears Henry (The name was changed for confidentiality reasons) was a very active man, who does many sports like bicycling. He was part of any cycling club. He took part in local cycling competitions and also managed his athletic body by going to the gym regularly.

Henry was also a big fan of swimming and loved going out in the pool.

Unfortunately many of the things henry does such as cycling he had to give up because of the illness he had gotten.

From the observation it looks like Henry has issues with the left aspect of his body, he can't properly move his still left body because of the damaged left aspect of his brain. Which results in problems in doing each day tasks.

Henry also isolates himself from modern culture and he doesn't want to meet his friends because he feels ashamed of his remaining body not operating. Another problem is going for walks or to the shop because of his still left leg.

Challenges

The problem the client faces is dressing and ability to move himself and day-to-day tasks your client could be doing along with his left hands and also still left leg.

Competence

Client exhibited competences to doing tasks like showering brushing pearly whites aren't problems for the client. Henry eats food normally since he is right handed or with going to the toilet.

Current Measures

My customer needs improve dressing, changing clothes, treatment exercises. Henry needs aid in making breakfast, dinners and suppers. Aswell he also needs someone they can trust to do shopping from a set of things henry wants bought.

Realistic Advice

To improve the client's life quality, he needs treatment exercises and physiotherapy. The exercises will need place in the local health club for aqua category aid in balance and muscle power which can also help Henry with getting together with new people and renewing old friendships. Henry requires mental assistance and drive to make fighting with the condition easier.

Personal Learning

Working with Henry helped my communication skills, that will definitely help me in the foreseeable future working with other clients. Supporting Henry at the Gym better my physical form and shape which will help me, to help others with their walking disabilities etc. The most important thing is too motivate clients and present them expect them to remain strong and attack the condition they can have.

The Customer is Mary, aged 18 - Cultural Isolation.

Observation and Customer Background My customer Mary (The name was evolved for confidentiality reasons) very young who suffers from sociable isolation because of schizophrenia, paranoid schizophrenia to be specific. Mary was raised in a home where one of the parents experienced schizophrenia. Mary grew up in an exceedingly specific environment, client's Mother who was the the one which experienced schizophrenia resided in her own world and didn't service or show affinity for her little princess, she often put in her amount of time in a healthcare facility. From an interview completed with the client's dad is due that Mary from an early on time fled into a illusion world in the old age of his life Mary commenced to isolate from culture, and its associates with peers or other people. Client often forgets words and mixes the order of words in sentences. Mary hears voices that comment on her behaviour and this tell her to do things.

Challenges A challenge for your client is meeting with the family and with people appearing out of the home. Big challenge for the client is to changing their diet, medications she actually is taking client decelerate metabolism and rises appetite which can lead to putting on weight.

Competence Client is fully mobile because of this initiative shows to execute many mobile duties, such as walking, dressing, eating, personal cleanliness such as bathroom not constitute a challenge for the client.

Current Procedures My client requires an Associate which is to provide for Mary. She needs an assistant for trips to the parks to help her state of mind of security on the outside and the associate will be participating in with her for psychotherapy. Mary also needs support and understanding from relatives and buddies.

Realistic Suggestion My suggestion is I'd like help to enhance the life for Mary. I Suggest The client needs physical exercise to improve metabolism, also needs to sleep well and should not undertake too many obligations. Your client by the disease is a lot weaker psychologically for what shouldn't race with others in anything for example competitions. And most importantly she needs psychotherapy to help reject the whole disease thing and change the convinced that my client understands that what her work is where the symptoms of the disease look and whether she will take the medication is determined by its functioning

Personal Learning Long dialogue with Your client has better my communication skills, supporting Mary sharpened my senses to see details, ready me for different circumstances improved my thoughts to understand the dangers before they even seem so that in the foreseeable future I can protect my entire life and the clients.

Mary trained me that you cannot dispute with the individual, you cannot irritate a patient since it can only anger patient and aggravate health. I discovered to make friends be pay attention to Mary. I hope that Mary will also have a standard happy life, I wish.
